Terms

• Must be 21 or older to join

• Submitting this form authorizes Story Hill BKC to charge my credit card for Wine Club subscription selected

• Valid ID is required for wine pick up

• 2 names are allowed per 1 Wine Club membership

• Rewards points will be earned with every Wine Club charge. No other discounts can be applied

• We will hold wines up to 90 days. No refunds or exchanges

• Deadline to be included in the next Wine Club is 2 weeks prior to Release Day
